The Risk Management office identifies and mitigates risk exposure. The following information covers how to fill out and submit the proper forms for film shoots on both private and public property, and the use of third party vendors for film projects in order to obtain Certificates of Insurance (COI).

Certificates of Insurance
Important Information:
- Requests for Certificate of Insurance (COI) should be filed at least 5 business days prior to date of shoot.
- All requests for Certificate of Insurance (COI) must be faxed from EDC (617) 824-8817
- Approved Certificates of Insurance will be emailed, please ensure email and phone number are clearly legible.
Review all Certificates of Insurance to ensure ALL the information is correct:
- Address of location
- Contact person
- Date(s) of shoot
- Time of shoot
- COI’s for third party vendors correctly states full value of rental equipment
It is the responsibility of the requester to affirm information is correct and to communicate any issues or questions as soon as possible. In most cases, corrections can be made on the same day of notification.
Film Projects
A. Film Projects on Private Property
File an A3 Form for each location address. When filling out this form, please remember:
- Information needs to legible.
- Contact information: Student name, email address and phone number.
- Complete address for each location.
- Name of party granting authority.
- Period of time equipment will be used, include day of pickup and day of return.
- Signature of student, professor, safety and Equipment Distribution Center associate.
- Fax from the Equipment Distribution Center to: (617) 824-8589.
B. Film Projects on Public Land
Film Projects in the City of Boston or on Department of Conservation and Recreation land require a permit. Permit requests should be filed a minimum of 2 weeks before date of shoot.

You must also file an A2 form for each location used for shooting. When filling out this form, please remember:
- Information needs to be legible.
- Contact information: Student name, email address and phone number.
- Complete address for each location.
- Name of party granting authority. Permit does not have to be obtained prior to filing an A2 form. However, a certificate of insurance (COI) cannot be released until permit has been granted.
- Period of time equipment will be used, include day of pickup and day of return.
- Signature of student, professor, safety and EDC Associate.
- Fax from EDC to (617) 824-8589.
C. Film Projects on Other Municipal Public Land
Most cities/towns other than Boston do not require a permit, however please remember to contact local police department or municipal authority to explain purpose of the project, location of shoot, date and time period. You must also follow any guidelines required by local municipalities. File an A2 form for each location using the same guidelines as noted above except contacting Emerson’s Government and Community Relations Dept. is not required.
D. Film Projects Using Third Party Vendor Equipment
A third party vendor is defined as any organization, company or individual that rents or supplies equipment excluding EDC. Copies of rental agreement should be filed with A3 or A2 forms. The period of rental should be clearly stated and within the dates of shoot as reported on A2 or A3 forms. Replacement cost for each item is required; support must be provided by third party vendor and filed with A2 or A3 forms.
